Cannabidiol Interactions with Medications, Illicit Substances, and Alcohol: a Comprehensive Review
In addition, the metabolism of certain BZDs involves cytochrome P450, leading to the alcohol-induced changes in metabolism described earlier in this article. In addition to CYP2E1, at least two other cytochrome enzymes that metabolize various medications (i.e., CYP3A4 and CYP1A2) also can break down alcohol (Salmela et al. 1998). Moreover, the amounts of various enyzmes of the cytochrome CYP3A family (including CYP3A4) can increase from alcohol consumption (Niemela et al. 1998). Thus, potential interactions also exist between alcohol and medications metabolized by these cytochromes.
So, mixing too much alcohol with any acetaminophen (or too much acetaminophen with any alcohol) can make removal of this substance even more difficult. (Stimulants are drugs that activate your central nervous system.) Examples of prescription stimulants include Vyvanse (lisdexamfetamine dimesylate) and Focalin (dexmethylphenidate). Taking antipsychotics such as Abilify with prescription stimulants could increase your risk of side effects from Abilify and stimulants. Certain medical conditions or other health factors may raise the risk of interactions with Abilify. Before taking Abilify, talk with your doctor about your health history.
How alcohol and acetaminophen affect your liver
But this doesn’t mean that vitamin interactions won’t be recognized in the future. For this reason, it’s still important to check with your doctor or pharmacist before taking any of these products while taking Abilify. Before taking Abilify, tell your doctor if you take quetiapine.
Alcohol may have contributed, given the improvement following discontinuation of ethionamide and a 2-week cessation in alcohol intake. Early in vitro studies suggested that metronidazole or its metabolites inhibited liver alcohol dehydrogenase (67–69). A more recent rat study found that metronidazole and alcohol increased intracolonic acetaldehyde levels, without altering blood levels (70). This review may also be helpful in assessing a patient who presents with an adverse drug effect which may or may not have been due to an alcohol and antibiotic interaction. If a reaction does occur, this review provides mechanisms and symptom complexes potentially allowing for a more efficient diagnosis. Certain opioid pain medications (e.g., codeine, propoxyphene, and oxycodone) are manufactured as combination products containing acetaminophen. These combinations can be particularly harmful when combined with alcohol because they provide “ hidden” doses of acetaminophen. As described in the previous section, alcohol consumption may result in the accumulation of toxic breakdown products of acetaminophen.
Other Medical Problems
Due to the lipophilic nature of CBD and THC, these compounds bind to these receptors and exert several pharmacological activities. In particular, CB1, TRPV1, and 5HT1A are thought to be related to psychosis, anxiety, and pain, respectively. As reported by several researchers, CBD appears to have minimal analgesic activity25.

Interaction with certain antidepressants
Chronic heavy drinking, however, can increase CYP2E1 activity up to tenfold, resulting in a substantial increase in the proportion of alcohol that is metabolized by this enzyme rather than by ADH (figure 3) (Lieber 1994). The effect of lower levels of alcohol consumption on CYP2E1 activity is unknown. Because CYP2E1 also metabolizes several medications, alcoholics, in whom CYP2E1 activity is enhanced, exhibit increased metabolic rates for those medications when they are sober. When those alcoholics are intoxicated, however, the alcohol in their system competes with the medication for metabolism by CYP2E1. With many medications, increased or decreased metabolic rates can have adverse or even fatal consequences. With increased metabolic rates, the medication’s concentration in the body may be too low or may decline too fast for it to be effective.

Levofloxacin, moxifloxacin, and trovafloxacin had improved efficacy in alcohol-fed rats compared to alcohol-free rats (24). All antibiotics were equally effective at improving survival and had improved efficacy in alcohol-fed rats. Moxifloxacin demonstrated dose-dependent survival in rats who did not receive alcohol, further supporting increased efficacy in the presence of alcohol, as the lower dose was equally as effective in alcohol-fed rats.
